Privacy Complaints

At the WVDE, we take your concerns and complaints about privacy violations very seriously. If you believe that your student’s records have been disclosed to someone who should not have access to them, we want to know.

When you submit a complaint about a privacy violation, we will begin investigating the issue right away. We will work closely with you, school staff, and district officials to make sure we have all the information we need to figure out what happened, why, and what needs to be done to fix it—both at the moment and in the future as well. We know you place a great deal of trust in us to get it right when it comes to student data, and we work every day to meet your expectations.

Submit an Incident Report

We've created a brief form for you to share your reports of privacy incidents and violations with the WVDE Office of Legal Services and Accountability. Parents and students older than 18, please complete the Parent/Guardian and Eligible Student Concern Form to tell us what happened with your child's (or your own) records or data. Citizens, please complete the Citizen Concern Form to tell us about an incident involving a student you know.
