Email Application Forms Updated

The educator and student email application forms have been updated. New language has been added to the educator form to clarify legal issues. A field for WVEIS ID has been added to the student form. This is a required field. School and county email contacts can now search for students by entering a WVEIS ID. This will prevent duplicate accounts. Please make sure you use these updated forms and discard any old forms you may have printed.You can access the new forms here: http://wvde.state.wv.us/technology/network/emailpdf.php.
