The school or county e-mail contact must login at http://access.k12.wv.us:1026  and enter the names and other information to start the process for a new e-mail account to be created.  Mail the teacher/administrator/employee forms to the WVDE.

The student application forms are retained at the school or county level and NO LONGER have information about sending the student form to the WVDE.

The student form should read as follows:

DO NOT SEND THIS STUDENT E-MAIL APPLICATION TO THE WVDE.
This document should be retained on file in accordance with the policies and procedures of your school and county school system. This document should be transferred with the records of the student if transferred to another school or county school system within the state of West Virginia.

The school or county e-mail contact MUST enter the complete name of the student (as it appears in WVEIS records), WVEIS number and the projected year of graduation into the e-mail account management system. [http://access.k12.wv.us:1026]
