General Information about E-mail Accounts

Managing the access.k12.wv.us e-mail accounts is an important and time consuming job for the Internet contacts at the schools and counties, the West Virginia Department of Education, Office of Instructional Technology and for WVNET.

Managing the thousands of accounts becomes very difficult when names are not uniquely identifiable. Applications for e-mail accounts must contain a first, middle and last name and complete information on the rest of the form or they will be returned unprocessed.

The applications should be one page, double sided. One piece of paper is much easier to file and eliminates the possibility of two pages becoming separated.

The Acceptable Use Policy (AUP) training required on the application does not require actual hands-on e-mail and browser training. The application may be submitted after the AUP training is completed; the practical training may be scheduled later.

The training must be accomplished using the current Policy 2460, dated September 9, 2001, which is accessible online at http://wvde.state.wv.us/policies/p2460.html and district or school policies related to the use of the Internet and associated technology.

The application form is a legal document and the preprinted information must not be altered.

Printed information must be legible to avoid errors when the accounts are created.

Dates must include the month, day, and year.

The full legal name is desired. Initials should not be used in place of a name.

Student birth date must NOT be the current year.

Current grade level and graduation year must agree on student forms.

Date of training must not be later than the signature dates, and should be within a reasonable time frame prior to submission to reflect current training.

Items 4-7 should all be checked "yes" (original checkmarks preferred).

Student signatures should be in cursive writing when appropriate for the age.

Signatures must be original on each individual form, not photocopied or stamped.

Contact names must be accurate and up-to-date.
