Surveys are administered annually for students with disabilities as they exit our school system and again one year later. This information provides valuable information and contributes to changes in the system. It is important to keep up-to-date contact information on students before they graduate so they may be contacted to complete these surveys. These contacts should include home and cell phone numbers, email addresses, and street addresses of both the students and their families.
As districts prepare students for graduation, it is important to raise their awareness of the importance of completing the post-school survey when they receive it in the mail. In Wisconsin, educators and students got creative by producing this video, which you may want to share with your students.
