Applications for Certification
in West Virginia

The Office of Professional Preparation is happy to provide you with the forms necessary to make application for certification in West Virginia. Below, you should find the information necessary to request licensure in our state. Select the appropriate form and print it. A valid email address is required for all applications. Upon completion, mail the application, appropriate fee, and all other pertinent information to:

Processing fees changed beginning July 1, 2014. All applications postmarked after July 1, 2014, must include the new application fees. Please click on New Processing Fees for a list of the applicable fees.

Additional Endorsement Based on Content Proficiency (Guidance Document)

***The current Form 8 may not be used to add an endorsement under the new WVBE Policy 5202 provision regarding passing a Praxis exam. Any Form 8 submitted requesting the addition of an endorsement based on passing a Praxis exam will be denied. As per the OPP policy, any fees paid are non-refundable and will not be able to be transferred/used for subsequent or future applications submitted.

A Form 7 must accompany ALL initial applications. In addition to granting the West Virginia Department of Education permission to review records received from the WV State Police and FBI, the Form 7 may be used to request that an applicant's WV State Police fingerprint results be forwarded to a WV County School System. ** Original with signatures required. No faxed forms or copies are acceptable.

Form 4B Character Recommendation

Submit a Form 4B, character recommendation, with your application if you are not currently employed by a West Virginia County School. If you are employed in an educational setting other than a WV public school, please complete the "Educational Experience" section and obtain the recommendation of your immediate supervisor. However, if you are not employed in an educational setting, you may complete the "Non-Educational" section and obtain a recommendation from an employer or community leader. ** This form is not required if a college or university is signing the recommendation portion of forms 19, 20A or 20S or 20SA or 20T.

Moving Expense Reimbursement Use this form when applying for reimbursement of fees associated with a move after employment has ceased in a West Virginia County due to reduction in force and you have been hired in another West Virginia County.

ASHA, NBCC, WVBEC, NASP, or NBCSN Enrollment & Completion Fee Reimbursement

Use this form when applying for reimbursement of fees associated with completing ASHA, NBCC, WVBEC, NASP, or NBCSN certification.

Applications received in the Office of Professional Preparation by the annual deadline of September 15 will be processed after September 15 of that year. Applications received between September 16 and December 31 will be denied. Applications may be submitted between January 1 and September 15 of each year.

The fee reimbursement is not awarded on a first-come, first-served, basis. Selection is made based upon official ranking criteria. Please refer to information in the application concerning these ranking criteria. No applications will be processed until after September 15 of the year.

Use this form when applying for the salary supplement associated with certification through ASHA, NBCC, WVBEC, NASP, or NBCSN.
