First Time Applicants for Certification

Information for First-time Applicants

Under W. Va. Code section 18A-3-10, applicants for initial licensure in the state of West Virginia are required to undergo a criminal history investigation completed by both the West Virginia State Police and Federal Bureau of Investigation. Therefore, it is necessary for ALL first time applicants to complete a fingerprint/background check as part of the initial application process.

In order to complete a fingerprint/background check applicants have two options. Applicants may register and schedule a LiveScan appointment (electronic fingerprinting) for one of the locations throughout WV. In order to schedule an appointment, applicants need to contact L-1/Morpho Trust via their website http://identogo.com/ or via telephone at (855) 766-7746. Another option is to request fingerprint cards from our office at the WVDE. Two cards will be mailed to you. Applicants will need to complete both cards, have their prints inked on the cards, sign, and submit to L1/Morpho Trust to the Springfield, IL facility stated on the submission form (WV Card Scan Form).

Payment of $45.35 is payable to L-1/Morpho Trust directly. Do not include the fingerprint fee with your application to the WVDE.

If you are an out-of-state applicant, you will need to request fingerprint cards from our office at the WVDE. Two cards will be mailed to you. You will need to complete, have your prints inked on the cards, sign, and submit to L1/Morpho Trust to the location on the submission form.

Fingerprint/background checks completed for or through other agencies/systems/schools cannot be used for WV licensure application. All fingerprints/background checks must be completed following one of the above-mentioned processes.

Form 7

A Form 7 must accompany ALL fingerprint cards and initial applications. In addition to granting the West Virginia Department of Education permission to review records received from the WV State Police and FBI, the Form 7 can used to request that an applicant;s WV State Police fingerprint results be forwarded to a WV County School System.

Form 4B

Use the Form 4B to obtain a character recommendation if you are not currently employed by a WV County School. If you are employed in an educational setting other than a WV public school please obtain the recommendation of your immediate supervisor. However, if you are not employed in an educational setting, you may obtain a recommendation from an employer or community leader. **This form is not required if a college or university is signing the recommendation portion of forms 20A or 20T or 20S or 20SA or 19 or 19A.

Use this form to obtain an institutional recommendation when applicant has completed an approved administrator preparation program in the state of West Virginia.
Use this form to obtain an institutional recommendation when applicant has completed an approved administrator preparation program in a state other than West Virginia.

Form 20T

Use this form to obtain an institutional recommendation when applicant has completed an approved teacher preparation program in the state of West Virginia.

Form 20A

Use this form to obtain an institutional recommendation when applicant has completed an approved teacher preparation program in a state other than West Virginia.

  Use this form to obtain an institutional recommendation when applicant has completed an approvedstudent support preparation program in a state other than West Virginia.

Instructions for Submitting an Initial Application

Download and complete all applicable applications.

Complete just one Information Sheet and attach all applicable applications.

Complete a fingerprint/background check through L-1/Morpho Trust.

Download and complete a Form 4B if you are not employed by WV county board of education. **See Form 4B above. (Some applications require Superintendent's signature)

If you are employed in a educational setting, please obtain the recommendation of your immediate supervisor. However, if you are not employed in an educational setting, you can obtain a recommendation from an employer or community leader.

Download and complete a Form 7 (Release of Information).

Use this form to request that a WV county of board of education receive a copy of the results from your WV State Police background investigation.

Include original seal-bearing transcripts from all institutions attended.

If you are applying based on certification in another state, include a copy of the certificate issued by that state.

Pay online using a major credit card by visiting the online payment page.

Be sure to include the appropriate processing fee(s) for EACH application attached to the Information Sheet. **Processing fees increased beginning July 1, 2014. Please visit the fees page for a complete listing of fees.

DO NOT INCLUDE FINGERPRINT FEES WITH YOU APPLICATION. Fingerprint fees are payable directly to L-1/Morpho Trust.
